Abstract

Care delivery teams in Primary Care require a transformation to meet the increasing demands of healthcare and the population of patients served. Few examples exist of successful change on this level due to lack of attention to the human side of change. Change management principles identify and attempt to mitigate common mistakes made in failed change management efforts. This paper outlines a playbook to support Primary Care leaders through a successful care delivery transformation with specific attention to managing the human side of this change.
