Abstract
According to a study done by the National Health Policy Institute (NHPI) "almost 20% of hospitalized Medicare beneficiaries are readmitted within 30 days, of which over half are potentially avoidable" (Baron 2013). Hospital administrators are continuously struggling to reduce readmission rates, which have detrimental impacts on hospitals and patients. This project adapts the Deming Management Method to address readmission rates via an education model that can enhance transitional care. The model will be in a seminar format using PowerPoint slides to deliver the approach with a modification of Deming's model. Seminars would be conducted for physicians, nurses, case management, nutrition, and pharmacy department directors. Using this model could help hospitals reduce readmission rates and increase patient health outcomes.
